CAR SOMETIMES JERKES AFTER SHIFTS OR TAKING OFF

Ok quick question guys just want you guys opinion on an issue, ok when shifting through the gears it sort of jerks after I put it in gear. It isn't as bad when it's cold but does it more when it warms up. What do you guys think about this? Thanks for all the help in advance guys....

yes I have tried this and it is more successful when the car is cold but less effective after it warms up
