I've read of cases where the EGR tube running to the exhaust manifold was restricted due to carbon build up. The tube had to be removed and cleaned out.

some cases with header installs, people cross-thread the egr tubing which leads to improper torquings and leaks. behind the engine near the heads are rubber hoses connecting the egr main tubing.

My brother had this same code on his 96 Gt.
We found the small metal tubes coming off the EGR going to the DPFE were clogged with carbon.
We removed the carbon deposits by using a small metal brush on a cordless drill and a bit of crab cleaner.
