i really need help with my 95 mustang gt, UPDATE PLEASE LOOK

so i have a missfire or something is not right. feels like im not firing on all cylinders.
The car revs up choppy and as im driving off from 1000rpm-2000rpm it feels rough then it clears after
2k rpms but if i mash the throttle it feels weak, so its def making me lose some ponies. Also when the car is in
low rpm in 5th gear from 1000-1500 doing about 50mph the engine and my shifter shakes really bad, it didnt used to do this at all. The problem only used to occur only when the car was warm, cold it would run perfect, now the problem is there at all times :(

Things ive replaced;
Maf sensor
ACT sensor
Ect sensor
tps sensor(when i switched this, car ran good for about 4 days then prob came back)
cap & rotor
spark plug wires
Spark plugs look good
Tried different coils with a friend, didnt help
Put new K&N filter
timing is at 15 (no pinging)
fuel filter
alternator
dizzy is fairly new
o2s are about 6months old

What could be causing this guys? ive spent countless hours
trying to figure out what the problem is and i cant seem to fix it
its stressing me the hek out.

help would be really appreciated!!

What are the mods on the car?

Sounds like it could be the idle air control valve. though the fact it used to only happen when it was warm makes me think that it could be the PIP sensor that is actually in the distributor itself.

gt40 heads
94/95 cobra intake
e303 cam
cold air intake
24lbs injectors
76mm c&L with 24lb sampler tube
afpr set at 40psi
long tubes
msd 6al ( ran like crap before it so its not the issue)
pulleys
bbk 70mm t.b

I ran this setup for a while now and this recently barely started happening.

how old is the harmonic balancer? and distributor?Both are common issues with the 5.0's that are getting up in age.Has it been tuned after the top end was replaced?

Harmonic balancer is new, distributor is used but the insides look nice and new.
No tune, car ran perfect without it, what Im feeling is a missfire . Engine stumbles bad in low rpms specially in 5th

Sounds like a vacuum leak to me.

But why only when it was warm ? And now at all times? Today i noticed the charcoal canister had broken pieces of dried accuses in there. I cleaned out what i could, but i doubt that's the issue .

Check for intake/vacuum leaks. as metal heats up and gets cold, it expands and contracts. check your upper and/or lower intake gaskets. had this identical issue on a Z71 i used to own with a TBI motor and found that i had an intake leak near the drivers side rear of the intake. fixed the gaskets and no probs after that.

Mods have been there since January. This has been haunting me for 3 weeks. I noticed it after i cleaned my k&n and swapped fuel filters.
Since then, it would only occur when engine was warm. I've put a whole new k&n, maf and two fuel filters. This weird thing happened once. If was running like completely dog shit one day, so the next day i swapped out tps sensors with another one that i had sitting there. After that problem went away, i was able to cruise 5th at low rpms with no stumble, no rough acceleration or anything. Every now and then ill get a backfire if i randomly hit he pedal in 5th. It ran good for about maybe 2 days until one day i hopped in the car and tried to turn it one and everything was completely dead, no power when i turned the key forward. Just barely enough power with he.key back tothe car see my dash clock and my radio flicker. So i disconnected my battery for a about 5min and the car started right back up but along with the missfire. And its been there since. Idle also drops all the way to 500-250 then just rises back up and falls again at least three times before a steady idle also

I have heard mixed reactions about the C&L but leaning towards the "do not use" end of the scale honestly. I would try swapping it out or again I think at this point you could benifit from a custom tune.

Your PCM doesn't like the mods, some 94 5.0L's respond well and can run months untuned (mine did) but mine would randomly stall out and the CEL would come on under moderate-heavy accell. I'd get a tune first before doing any more diagnosing.
