What Can It Be?

After my car gets warm my clutch starts too act funny. When i lift off everythings good until i get too that last 2 to 3 inches of clutch and then i get this jerky jerky thing happening. this causes my car too shake violently, anyone know what this could be?

Warped pressure plate more then likely...I dont think a bad throught bearing would cause this would it anyone?
