94 gt engine problems. I need help!

I have a 94 5.0with a c4 in it. wnenever i put I move it out of park into gear it jump and the rpm's lower then catch back up. but after the car warms up and I try to move it onto gear it will do the same thing but the rpm's drop too far for the car to stay running and it dies unless I apply the gas while im moving it into gear. the rpm's also jump up and down as it idles after the car warms up. Doe3s any one know what may be wrong with my car?

Couple questions... not to insult your intelligance, but are you sure your car has a C4 and not an AODE? I only ask because I know some people like my father thinks all Ford automatics are C4s. He still thinks his current car has a C4 even though Ive told him a million times its a 4R70W. Anyways.. so if you do have a C4, who did the install? Did it do this before the install? You might want to check the stick about idle surge because otherwise it sounds like youre a perfect candidate. My car does the same thing and Im pretty sure its the TPS sensor but after working on other peoplescars for 8 hours I usually dont have the motivation to work on my own.

yes im sure its a c4. I just recently bought the car and got the tranmission rebuilt. Im not sure if it done it beforethe swap or not.

would that also be the reason for it to jump ad die when I go form park into gear
