can't figure out why tensioner is throwing belt off?

hey, my tensioner doesn't seem to be lined up with the hb and the water pump. I thought it was because of the bracket (long story) so i bought a new bracket and it still doesn't work. what is happening is the tensioner looks a little off center compared to the other pulleys and it is making the belt go away from the block and off the tensioner pulley. anyone have this same problem? i've already gone through a 60 dollar belt and a 50 dollar bracket...

Sounds like your belt tensioner is starting to come apart... Does it look like the little black pulley is sitting correctly on
the tensioner, but the tensioner is sitting crooked? You probably need to replace your serp. belt tensioner..

well, i thought it was that too but I put another tensioner on (new) and it still was thrown off. I took a straight edge and it seems like my alternator pulley is out too far? I can't seem to figure out why though since there isn't anyway to screw up putting that on.
