First, for MAC LTH will I need O2 Sensor extenders? Anything else needed?

Secondly, I removed the EGR as well. Only thing I will need is the little plate to cover hole correct?

Lastly, should I get a custom tune or just chip it? CAI, LTH, straight pipe exhaust.

Yes you'll need O2 wire extenders.

You'll need a tune to disable the EGR so you don't throw a CEL.

I wouldn't bother with a tune with those simple mods.
