Transmission Help

Well, after my car being in the shop for about a month I got back Friday afternoon. Car drives excellent up until last night. When I was shifting through the gears and this is me drive the car normal, the felt the clutch cable get loose. Then about 10 minutes later, it got loose again to the point where I couldn't shift the car. Luckly, I was near by another friend who owns a shop. Threw it on the lift and what do I see, the throw out bearing split into pieces. Now all my years on working on cars, I never seen this before. I do remember the guy telling me he adjusted the cable for me. And also it was to stiff but I didnt think anything of it. Here are a few pictures of what I saw. any helpful advice would be great.

The bearing coulda been on its way out already...

What work was being done at the shop this car was at for a month?

The King Cobra clutch is brand new and the transmission is rebuilt. Everything maybe has 200 miles on it.

sounds like a manufacturing defect in the TOB... ... gotta take the clutch fork out which means the tranny has to drop... id also mail the damn thing back to the manufacurer... if its not a FRPP heavy duty TOB, id invest the extra money into one...

It was the throw out bearing that came with my FRPP King Cobra Clutch Kit. Should I put in another Ford one in or is there a company that makes better ones?
